Amend Forms
Amending forms for Incorrect Social Security Number
- Press Remove on the form(s) that need correcting.
2. In the Dashboard, you will see the amount of forms removed.
Press E-File and follow the steps.
By doing this, you are removing the incorrect form(s) from SURI.
3. Then, go to Add Forms and create a new one with the correct information.
4. E-File the correct form(s).
- Note: At the end you will have 3 confirmation number for the same tax payer, one for the original form, one for the removed form and another for the corrected form.
Amending forms for Incorrect Basic Information or Compute Errors
- Go to Amended Forms.
- Press Amend on the form(s) that need correcting.
3. Enter the reason for change.
4. Press Continue.
5. Edit the information that need correction and press Save.
6. E-file the corrected form.
Creating amend forms using Excel
1. Go to Add Forms.
2. Select Import Informative Information from Excel (AMENDED).
3. Download and fill the template.
*Make sure to enter the Reason For Change.
4. Import the Excel.
5. In the Dashboard, you will see the amount of forms created.
Press E-File and follow the steps.
